Free Flu Immunization Program 2019
Alachua County Public School families will once again have the opportunity to protect their children and other family members from the flu.
The annual Control Flu program is being offered to all ACPS students. Elementary students will receive the FluMist nasal spray. Secondary (middle and high school) students will receive flu shots. All immunizations are FREE and will be provided at school during the school day.
To take advantage of the program, parents must sign and return consent forms to their child’s school. The deadline to submit a consent form for the secondary school immunization program is September 25. The deadline for the elementary program is September 30.
Separate forms for both the elementary and secondary programs are available on this site. They include a letter to parents, the consent form, a vaccine information sheet and the federally-required privacy notice.
