Parents can once again protect their families from the flu by registering their child for the FREE FluMist immunizations that will be offered at all local schools this fall through the annual Control Flu Alachua County program.
FluMist is a gentle mist that’s sprayed into the nose. It’s a safe and effective way to prevent flu in children and families.
There is NO COST to families to participate. However, if your child has health insurance, the Health Department is required to collect the insurance information and bill the company for the vaccine. However, there will be no co-pay or deductible due! Your health insurance information will remain confidential. Children without insurance will receive the vaccine for free through the Vaccines for Children Program.
On this page you will find links to a Vaccine Information Statement about FluMist and a Notice of Privacy. You’ll also find the required consent form. If you’d like to take advantage of this free immunization, please download and return the form to your child’s school or have your child give it to his or her teacher. You can also fax it to the Health Department at (352) 334-7947 or email it to SLIV@flhealth.gov.
The deadline to return the form is OCTOBER 5.
