NEW STUDENT ONLINE ENROLLMENT

Online application for the 24-25 school year opens up on April 24th

 

To make sure your student is fully enrolled, please see the necessary items listed below:  

Required for enrollment-

  • online application at    www.sbac.edu/enrollment
  • original documented proof of name and age 
  • up-to-date Florida immunization record (DH 680)
  • Florida physical (DH 3040) less than one year from the first day of kindergarten
  • parent photo ID
  • TWO current proofs of address - Accepted proofs are: electric, gas, cable, internet, lease, mortgage, homestead exemption

 

If you do not have two accepted proofs of address in the name of the parent on the birth certificate and are choosing other living arrangements, an affirmation of address may be obtained at the Office of Student Assignment.  For an affirmation of address you will need the your driver's license, two proofs of address in the name of the person you are staying with, and a signed letter stating you and your children are living there.
